MyChart is a free, secure online tool that gives patients easy access to their health information, any time, from anywhere. Patients can view test results and other clinical records, enter personal health information, share their records and securely message their physician or healthcare professional.
Online account enrollment is available for people wishing to sign up for MyChart, without having to visit a participating hospital in person. Visit https://healthrecordsonline.ca/register in order to enroll for a MyChart account (if they are over the age of 12 and and are requesting the account on behalf of themselves).
Patients or a substitute decision maker may also register for their MyChart account during their next in-person appointment at Woodstock Hospital. In order to register, the patient or substitute decision maker will be required to provide a valid email address and Ontario Health Card Number.
Immediately after providing this email, MyChart will send an email from the email address MNyChartOntarioWest@hhsc.ca with a link to MyChart and a four digit PIN to complete the registration. Patients will be required to register using other identifiers, including: name, birthdate, gender and Ontario Health Card Number.
MyChart allows patients to:
- View their medical information online, including lab results, medication lists, pathology reports and more
- Share their records with friends, family and healthcare providers who they delegate access
- Input and manage information in their personal health record, such as medications, allergies, blood pressure and weight. This information can be viewed by healthcare providers who they allow access to their MyChart
- Securely message with those who they have given access to their MyChart account
